Definitions first:
Rheumatoid fever and rheumatism:
Same thing; a collection of symptoms describing joint or arthritis pain. Seen mostly in historical medical texts.

Rheumatic fever is a specific condition caused by antibodies produced by a person in response to Streptococcus infections (most notably Group A – Strep pyogenes).
